Tips on Completing the Survey

OEPA Premise Number
This is the 10 digit number that Ohio EPA assigned your facility. It can be found on your permit and on most correspondence from Ohio EPA and DOES. It begins with 14. Example- 1401010001 

OEPA Source Number
This is the 4 character label that Ohio EPA uses to distinguish one source from another. It begins with a letter and ends with three numbers. Example - B001, K002, P901, etc. 

Person Completing Form
Please give the name of the person we can contact should we have questions about your submittal. If this person is a consultant, you may want to give us the name of the consultant along with the actual facility contact to avoid later confusion.

Company ID
This is the name that you use to identify a source. Examples - boiler #2, press 8, paint booth 1, unit 6, etc.

Actual Annual Production
This refers to the amount of finished product that was produced last year (for processes). Example - If you're manufacturing a chemical, report the amount of that chemical produced, not the amounts of raw materials that were used to make the end result. 

Percent Operation Per Quarter
This confuses a lot of people. Take the total amount of time a source operated last year and figure out the percentage of that time that the source operated for each quarter. If the source operated on the same schedule all year, the percentage each quarter would be 25%. Make sure all four quarters equal 100%. 

VOC Content / VOC Density / Coating Density 
If you have a coating line and you want to determine the composition of a particular coating, first determine if the coating contains water and/or solids. if the coating is 100% solvent, you can get the density from the MSDS. If the coating is not all solvent, you need to distinguish between VOC content and coating density. The MSDS will tell you the coating density which is how much one gallon of that coating weighs. The VOC content, then, is the weight of VOC in that one gallon of coating minus solids and water. 

Last Test Date
This only pertains to you if you have had a stack test witnessed by a representative of DOES or Ohio EPA. Internal audits and engineering studies are not considered recognized compliance tests. If the source has not been tested, leave the space blank or write "none". 

Comments 
In the past, the questionnaire forms had a space reserved for comments. The updated forms no longer have adequate space for comments. Please include comments on a separate sheet or on the back of the form.
